fix(agent-status): set Claude Code blocked when a permission prompt opens (#721)

The installed hooks set blocked only from Notification[permission_prompt], which Claude Code sends once the prompt has waited about six seconds with no keystroke. Until then the session read active, and it never turned blocked while keys were pressed in the prompt. The installer now also maps PermissionRequest to blocked, which fires as the prompt opens. Notification stays for the sandboxed network prompt, which PermissionRequest does not cover.

With Status reset set to On Enter, Esc or Ctrl-C in the owning pane now clears blocked: cancelling a prompt fires no hook, so the status stayed up with nothing waiting.
