Recover from switching to an account whose OAuth grant has died (#536)

Switching to a stale remembered account froze the app: UserMenu dimmed the
page and disabled pointer events before the switch, and never undid it when
the switch failed.

- UserMenu restores the page on any switch error.
- An ACCOUNT_SESSION_EXPIRED switch now sends the user straight back through
  OAuth for that handle instead of leaving them stuck.
- useSwitchAccount refetches the session on error, so an account the server
  dropped leaves the menu.
- AuthService.checkSession treats a revoked / unrefreshable / invalid OAuth
  token as an expired session (null) rather than throwing, so the switch
  route returns 401 and forgets the account instead of a 500 that kept it.
