chore: require mp4ff v0.59.0 for range-checked deblocking offsets

mp4ff v0.59.0 makes avc.ParseSliceHeader reject out-of-range disable_deblocking_filter_idc, slice_alpha_c0_offset_div2 and slice_beta_offset_div2. With v0.58.0 hi264 accepted any offset, and frame.Deblock indexes its 156-entry alpha, beta and tC0 tables with qp + 52 + 2*offset, so an offset of 40 or -40 at QP 26 panicked with an index out of range. Such a slice now fails with a parse error.

TestDecodeRejectsOutOfRangeDeblockingOffset builds a one-macroblock CAVLC IDR with the offsets written by hand. With v0.58.0 the three out-of-range cases panic in frame.Deblock, while the in-range +-6 cases decode, which shows the slice is otherwise valid and reaches the filter.

The rest of v0.59.0 is MP4 box handling. hi264-mp4-extend builds its output fragment from the samples, so the top-level boxes that DecodeFile now keeps with a fragment do not reach its output.

Verified with go test ./... (including the golden FFmpeg checksums), golangci-lint, tools/verify_hi264gen.sh (12/12) and tools/verify_pskip_extend.sh (5/5).
