Add command and response details to SMTP exceptions (#1748)

- Added a public SmtpCommand enum identifying the SMTP command being
  processed when an error occurred (replaces the private nested enum).
- SmtpCommandException: new Command and ResponseText properties and new
  constructors taking an SmtpCommand and SmtpResponse.
- SmtpProtocolException: new Command and LastResponse properties. The
  last response is reset whenever a new command is queued, so for
  pipelined commands LastResponse indicates which queued command the
  server responded to before the error.
- Added SmtpServiceNotAuthenticatedException (derives from
  ServiceNotAuthenticatedException) with Command, StatusCode and
  ResponseText; thrown when the server responds with 530.
- The unexpected-disconnect message no longer includes a stale response
  from a previous command.
- Updated/added unit tests and the 5.0.0 release notes.
