Fix SOCKS clients spinning forever when the proxy disconnects mid-reply

Receive/ReceiveAsync return 0 when the proxy server closes the
connection, but every read loop in Socks4Client and Socks5Client simply
retried, so Connect/ConnectAsync would spin in an infinite loop if the
proxy disconnected in the middle of a reply.

- Added ProxyClient.ReceiveExactly/ReceiveExactlyAsync helpers that
  throw a ProxyProtocolException with ErrorType UnexpectedDisconnect on
  EOF, and used them for all SOCKS4/SOCKS5 reads.
- Added ProxyProtocolException (string, ProtocolErrorType[, Exception])
  constructors.
- Replaced the 'do we care about BND.ADDR and BND.PORT?' TODOs with
  comments explaining that these fields are only meaningful for BIND and
  UDP ASSOCIATE requests (which are not supported), so they are ignored.
- Added a DisconnectingProxyListener test helper and SOCKS4/SOCKS5
  unexpected-disconnect tests (sync and async).
- Updated the 5.0.0 release notes.
