fix: Refuse duplicate OnXXX parameters

Treat each name of an OnXXX parameter field as a parameter of its own
and refuse a second SSE, session or stream ID, as for every other
handler. Report `sse, sse2 datapages.SSE` in datapages lint and gen
instead of generating a call that does not compile.

Refuse a repeat in separate fields as well.
The spec allows each parameter once.
