fix(codex): stop reading a question mark that opens quoted text as a question (#684)

the Stop hook reported blocked for a finished turn whose final message
quoted a footer hint such as `“? for shortcuts”`. The question check takes
any non-space character before the `?` as the word it ends, and here that
character was the opening quote. A `?` that directly follows a run of
opening quotes, brackets or emphasis marks at the start of a line or
after whitespace is now dropped before the check. A quoted real question
such as `"deploy now?"` still reports blocked, and so does a question
after a quoted word, as in `Did he say "yes"?`

fixtures pin the offending sentence with each opener, a real question
next to a quoted mark, and a question after a quoted word.
