Link publishers to user logins (#414)

* Link publishers to user logins

Adds publishers.user_id (unique) so a login can be tied to the
publisher it belongs to. The link is set on the publisher form, which
only offers users of the same congregation that aren't linked yet, and
the model rejects users from another congregation.

* Keep a linked user in the same congregation as its publisher

ActiveAdmin lets a master change a user's account. Once the user is
linked to a publisher, that would leave the publisher pointing at a
login from another congregation. The user model now rejects the change
while the link exists.
