* fix(gatekeeper): stop DID status scans from overlapping

The status timer used setInterval, so a scan that outlasted the
interval started another on top of it. Each scan holds the history read
lock, so stacked scans queued user resolves behind all of them and the
node never recovered without a restart. GC also started scans of its
own.

Callers that arrive while a scan is running now share it, and the
status loop schedules the next report only after the current one
finishes. A failed scan is logged instead of escaping as an unhandled
rejection.

checkDIDs takes the history lock per 32-DID chunk, not for the whole
scan; overlapping scans harm resolves by multiplying that contention.
