docs: rework the documentation-preview landing page (#363) (#364)

The generated landing page assumed a visitor already knew what the site
was. It opened with a bare list of version numbers and deferred the one
fact that orients a reader (that none of this is the released
documentation) to a note below the fold.

Restructure it around that distinction instead:

- A two-column layout borrowing Furo's proportions (same 67em collapse
  point and item spacing as the builds it links to), so moving from this
  page into a build does not feel like crossing between two sites. The
  sidebar carries the Qiskit logo and the orientation text; the main
  column carries the listing and a copyright footer matching the Sphinx
  builds.

- `main` and `stable/X.Y` merge into one "Official branches" section.
  Splitting them left `main` alone under a heading whose title, note and
  entry all said "development" and nothing more. Their labels also no
  longer claim a stable branch is released: it accumulates backported
  fixes after X.Y.0 ships, so what it documents is the next patch.

- The pull-request section now always renders. Its absence was
  ambiguous ("nothing is being previewed" reads identically to "this
  site does not preview pull requests") so the empty state says which
  label turns a preview on and links it. That also removes the need for
  an empty-site fallback, since the page can no longer be empty.

- Entries for pull requests are labelled with the pull request's title.
  This script only ever reads a directory listing, deliberately: it runs
  inside the publishing action's push-retry loop and must list previews
  that concurrent jobs added, including ones this run knows nothing
  about. The title therefore travels as a `.pr-title` file written
  inside the preview directory, which keeps the listing filesystem-derived
  and makes the title self-cleaning, as removing a preview deletes the
  directory holding it.
