Release 1.12.0: get_cache_root for a shared cache root, fast import (#76)

Add get_cache_root(name, *envkeys) for a cache root shared by several
packages: the first environment variable set to a non-blank value is the root
itself, else the platform cache directory for name. Unlike get_data_dir, it
never appends to the variable's value, which pyensembl relies on. OpenVax
packages use get_cache_root("openvax", "<PACKAGE>_CACHE", "OPENVAX_DATA_CACHE"),
as documented in the integration guide with the shared objects/sha256 layout.

Import pandas, numpy, and requests only where they are used, so
import datacache takes about 35 ms instead of about 350 ms. download.pd and
download.requests still resolve to those modules.

Also release #65's db_type mapping of numpy fixed-width types.
