Render an XML error without checking for to_xml (#3002)

`ErrorFormatter::Xml.format_structured_message` fell back to `to_s` for
a message without `to_xml`, but it is only ever handed what
`Base#wrap_message` returns: a Hash (or grape-entity's delegator around
one), which the ActiveSupport conversions grape.rb requires give
`to_xml`. The fallback could not run, so it goes.
