Cover the header versioner's last two branches (#2996)

`Versioner::Header#version_not_found!` guards `versions&.none?` for a
middleware built without versions, which only direct use of it does --
through the DSL a header-versioned API always declares them -- and no
spec did. A spec now passes such a middleware a request naming a
version.

`MediaTypeForAcceptCache` skipped an Accept header whose media type came
back nil. Its table is only read as `table[accept] || best_quality(...)`,
so a nil entry would fall through to the same answer, and no spec could
tell the skip from its absence. The table is now built with `to_h` and
`compact`, which keeps it free of nil entries without a branch.
