Name a nested array element by its own indices through a Hash scope (#2990)

With a Hash scope between two Array scopes (`a` Array > `b` Hash > `c`
Array), a validation error under `a[0]` was reported as
`a[2][b][0][c][1][d]` instead of `a[0][b][c][1][d]`.

`AttributesIterator#store_indices` handed each parent index to the next
`.parent`. That is the enclosing array only when array scopes nest
directly; here it gave `a`'s index to `b`, which then printed it, and
`a` kept the index its own validators last iterated over.

Parent indices now go to each `nearest_array_ancestor` in turn. There
always is one: `parent_indices` holds at most `array_depth - 1` entries,
and the target scope has that many element-iterating ancestors. So the
`break unless parent_scope` guard, which could never trigger, is gone.
