Redesign `MultiProgramRunner`'s per-item hook surface (#137)

* Redesign MultiProgramRunner's per-item hook surface around real QuantumPrograms

Replaces MultiProgramRunner's per-subclass `_get_items`/`_process_item_fn`/`_static_kwargs`/`_finalize`/`_item_key_fn`/`_shot_checkpoint_subdir_prefix` hooks with a smaller, generic surface (`build_program`/`reduce_program_outcomes`/`build_output`/`_build_output`) that a shared base-class worker function drives directly, consolidating per-runner boilerplate.

- Introduces `run_kwargs` as a base-owned constructor field, normalizing the `max_frame_limit` default and the `checkpoint_dir`-conflict check that were previously duplicated per subclass.
- Adds a cascade-guard resume check to the shared worker, so a resumed run with existing on-disk shot state resumes instead of raising.
- Adds a `Sleep` instruction to the trivial-counter test codepack for controllable per-shot timing in tests.
- Migrates all 20 test-double subclasses in `test_multiprogramrunner.py` off synthetic int-doubling/hand-rolled-checkpoint fakes onto real `QuantumProgram`s built from that codepack, exercising the real checkpoint/shot machinery directly instead of simulating it.
- `EdesignRunner`/`NoiseSweepRunner`/`FaultInjectionRunner` are not migrated onto the new surface yet and are temporarily non-functional until a follow-up change moves them over.

* Complete test-duplication audit for MultiProgramRunner-derived runners

Resolves the deferred audit of MultiProgramRunner's hook-surface redesign, closing a gap between the base class's own now-tightened design surface and the test suites of its three not-yet-migrated subclasses (EdesignRunner/NoiseSweepRunner/FaultInjectionRunner).
- Removes dead hook-surface indirection and 3 stale comments left over from the redesign, and trims redundant test-double classes/tests the redesign's own migration created in test_multiprogramrunner.py.
