renderers: size the input group wrapper along with the input (#890)

make_input_group emitted a bare div.input-group. Bootstrap 4 wants
input-group-sm or input-group-lg on that wrapper, so with size="small" the
input got form-control-sm while the addon kept its default height and the
two rendered out of line.

Rendered output only changes for fields that combine an addon with a
non-default size.

Fixes #128
