docs: correct Bootstrap 3 leftovers and explain is-valid on bound forms (#893)

The bootstrap_field docstring still documented has-error and has-success
as the defaults for error_css_class and bound_css_class. The actual
defaults are is-invalid and is-valid, and have been since the Bootstrap 4
port. It also claimed the label always ends in control-label; FieldRenderer
adds col-form-label, and only in horizontal layout.

The settings page now explains that success_css_class applies to every
field of any bound form, which is why a GET filter form shows a green tick
on first load, and gives both the per-tag and the project-wide way to turn
it off. That is the question in #276, which asks about behaviour we are
keeping.
