* Rename or remove a field in place in the password dialog

Fixes #132. With "Show all fields templated" every `key: value` line of an
entry becomes a form row, but the key could only be changed by turning the
option off, editing the text and turning it back on. The label of such a
row is now a FieldLabel: double-click, or Rename in its context menu, puts
a line edit in the label's cell of the form (the column widens for it);
Enter renames the field, Escape leaves it; the context menu can also remove
the field. PasswordDialog refuses a name another field already has and
says so in the status label, and re-checks which field is the OTP one
afterwards. Fields that come from the template keep plain labels: their
names are the template's, renaming one would only leave a stray key.

* A visible way to remove a field: × at the end of its value

The context menu on the label was the only way, and nothing showed it was
there. Each entry-derived field now carries a trailing edit-delete action
in its value line edit, the way the OTP field carries its warning.

The docs job fails on undocumented members; the review asked for the
SPDX lines the other sources carry.
