* Add password: folder and name inside the dialog

Adding an entry went through a QInputDialog for the name, prefixed with
the tree's current folder behind the user's back, and then the
PasswordDialog for the content. The dialog now names the entry itself:
setNewEntryLocation() shows a folder picker over the store's folders
(the current one preselected) and a name field above the password and
the template fields. The name is checked as it is typed (empty, taken by
an entry, a folder, or resolving outside the store via PathValidator) and
OK stays off with the reason in the status line; accept() creates the
subfolder a name like work/vpn asks for, since gpg does not, and sets the
entry path that on_accepted() inserts. Editing keeps the row hidden and
the name untouched: rename stays a separate action.

MainWindow::addPassword() is a call to setPassword(); the folder list is
walked once per dialog. tst_passworddialog covers the hidden/shown row,
the validation states and the accept path, with FakePass recording what
Insert() is asked to write.

* PasswordDialog: a typed .gpg suffix belongs to the file

Insert() appends .gpg, so "github.gpg" became github.gpg.gpg (the same
slip pass mv had). One trailing .gpg, any case, is dropped before the
name is checked.
