Restore server workflow state from declared types by default (#757)

`WorkflowServer` defaults to restoring declared event and state types.
The base `Runtime` keeps `JsonSerializer()`, so standalone workflows and
`Context.from_dict` resolve stored classes by import path exactly as
before.

`ServerRuntimeDecorator` uses `JsonSerializer(allowed_types=[])` when
the server has no configured serializer. Choose declared types, add
classes, or retain import lookup:

```python
server = WorkflowServer()  # Declared types
server = WorkflowServer(serializer=JsonSerializer(allowed_types=[MyClass]))
server = WorkflowServer(serializer=JsonSerializer())  # Import lookup
```

Handler responses include `result_unreadable`, and event streams return
422 for unreadable results. Purge on a handler whose workflow was
removed from the server sends a cancel to the run, logs a warning, and
deletes the row. The OpenTelemetry example uses `/api` paths and
documents event replay.

### Compatibility

The default changes only for deployments through `WorkflowServer`,
including DBOS and the appserver. Workflows that store undeclared
classes in the context store need an explicit server serializer. This
includes `AgentWorkflow` from `llama_index.core`, which stores its
memory buffer and chat messages.

For MyClass, restoration fails with "Class MyClass is not in the
serializer's allowed types. Pass JsonSerializer(allowed_types=[MyClass])
on the workflow or server to allow it, or pass JsonSerializer() to
resolve classes by import path."

Use import lookup or list the missing classes:

```python
server = WorkflowServer(serializer=JsonSerializer())
server = WorkflowServer(serializer=JsonSerializer(allowed_types=[MyClass]))
```

These options also read previously stored data when the classes remain
importable or registered. A workflow serializer override takes
precedence over server settings.
