docs(signals): A33 — a fallback-caught flight holds no transaction; a Loading reset moves the hold onto the boundary

Lifts the #3375 ruling (2026-09-12) out of INTERNALS into the spec and
records its #3459 extension (2026-09-15): the readers behind the fallback
still wait, so the boundary waits for what they wait on rather than
revealing them stale. Pins carry the ID (async-chain-supersession, the
#3459 test — previously cited A15); the two sites cite A33 in existing
comments; rules index regenerated.
