fix(OTPInput): refuse a rejected character instead of eating the digit under it (#1294)

A character the mask rejects is refused at `beforeinput`, so it never lands and
the digit the slot held stays where it was. It used to overwrite the slot, which
then got blanked while the hidden input kept the old digit, so the form
submitted a character the user could not see. A bulk insert that holds nothing
valid still blanks the slot, and the hidden value follows it now.

Deleting a digit fired `change` twice, because the backspace branch recomputed
the hidden value a second time on the slot the `input` handler had just
recomputed.

The first slot goes back to holding a whole code once the field is emptied by
hand, so SMS autofill works again after the user wipes what they typed.

The ceiling for `coreui.min.js` moves from 91 to 92 kB: `v6-dev` sat exactly on
it.
