feat(keymap): expose the stable pane token to custom commands as AGT_PANE_ID

`AGT_PANE` is the role at fire time and a swap or promotion changes it, so a command could not name the exact terminal it fired in. `AGT_PANE_ID` carries the same token the pane's shell holds as `AGTERM_PANE_ID`, read from the slot the role names, so an overlay chord gets the covered pane's token and a sessionless launcher gets an empty value.
