test: use testify.require where a failed check makes the test meaningless (#1809)

Converts testify/assert to testify/require in 12 test files: cases where a
later line indexes, dereferences, or type-asserts the value just checked, or
a later step only makes sense once this one succeeded. Also folds in a
couple of manual "if err != nil { t.Fatalf }" checks that were the same
thing spelled out by hand.

assert stays correct, and is left alone, for independent checks (mostly
table-driven tests), calls off the test's main goroutine (require's
t.FailNow() calls runtime.Goexit(), unsafe outside the test's own
goroutine), and EventuallyWithT/CollectT callbacks (testify defines no
require.CollectT).

Documents this rule in a new docs/dev/testing.md.
