feat(semantic): related lookups across sources and query applicability with resolution chains (#304)

- RelatedLookups(entities, schemaByCollection, selected) []Lookup: FK graph
  in both directions within a selected value's own source (ForeignKey when
  the selected column is a local FK, ReferencedBy when it's part of the
  table's primary key and another table's FK targets it), plus SameField
  lookups through mappings of the same entity field in other sources.
  Deterministic: same source first (by collection, then kind), then other
  sources by name.
- Applicable(queries, available) (applicable, notYet): matches
  ParameterDef.Meta against available semantic values; a required Meta
  parameter with no match lands the query in notYet with its missing
  fields; an optional or untagged parameter never blocks. When available
  holds two values for the same entity+field, the later one wins. Each bound
  parameter gets a human-readable Chain entry.
- Added SemanticValue.Provenance beyond the brief's literal sketch, so
  Applicable's Chain can render "(declared)"/"(inferred)" instead of a
  hardcoded label - documented as a deliberate addition in related.go and
  the PR.
- README worked example per function; pkg/semantic stays at 100% coverage.
