fix: a postfix increment's value is the value before it (#313)

Semantic analysis named the operand as the result of `x++` and code
generation repointed it at the saved copy afterwards, so anything reading
the result during analysis saw the incremented value:

    int r = (0, x++);   // 6, where C says 5
    int r = ({ x++; }); // the same

Analysis now names the saved copy from the start and code generation
increments the operand, which also removes the last place where the back
end rewrote an annotation mid-emission.

The PreOperation slot held that copy for one writer and one reader, and
both now use the result, so the slot and its accessors go with it.
