fix: reject the operand types C forbids for `&` and for switch (#314)

Taking the address of something that is not an lvalue compiled, so
`&(1 + 2)` and `&f()` produced a pointer to a temporary. The bit-field
guard next door already had the shape this needed.

A switch had no check on its controlling expression at all beyond
rejecting a function value, so an array, a double, a float, a pointer, a
struct or a function designator all reached code generation and compared
whatever they happened to be. C requires an integer type.

Both are constraint violations gcc reports, and neither was diagnosed.
