ETT-1265 deprecated table export (#310)

* ETT-1265 Otis: replace deprecated table export plugin
 - Proof of concept
 - Still need CSV support in the controller
 - Still need a way to craft the download URL with the correct filters (inside the controller)

* - Rebase against latest
- Use a cached filter URL to construct CSV download URL
- First crack at CSV format for HTDownload
  - This is missing critical data
- No tests yet

* Remove filter control event monitoring -- not needed for CSV download URL construction.

* ETT-1265 Finish CSV download export: match presenter fields, drop deprecated plugin, add specs

* Remove unused file_name param handling from CSV download, per PR review
