ETT-1184 Add the columns toggle to user management tables (#283)

* ETT-1184 Add the columns toggle to user management tables
- This adds the `data-show-columns` attribute to Approval Requests, Institutions, Registrations, and Users tables.
- The ticket did not call for Institutions, however I folded it in for consistency.
* Persist the state of hidden columns (and records per page for Downloads) using the Table Cookie extension.
* Cookify rows per page when displaying Downloads
