feat(utilities): fixed widths from $sizes, vertical gutter knob (#1155)

* feat(utilities): fixed widths from $sizes, vertical gutter knob

$sizes (1-12 x $spacer) is merged into the width utility, so .w-1 ... .w-12
ship next to the percentages. $grid-gutter-width is $grid-gutter-x and
$grid-gutter-y joins it: make-row() takes both and writes --cui-gutter-x
and --cui-gutter-y (the row gutter still defaults to 0, so nothing moves);
make-grid-columns(), the CSS grid gap and the card group read
$grid-gutter-x. $grid-gutter-width stays as a deprecated read-only
alias, because the shared docs engine reads it. Width page documents the
steps and the map; migration entry.

* chore: raise the utilities CSS ceiling by a quarter kilobyte

coreui-utilities.css lands 40 bytes over 20.25 kB with the twelve .w-*
steps; 20.5 kB, size work comes at the end of v6.
