refactor(OtpInput): render the cell through the form-control frame (#1126)

.form-otp-control declared 19 of the 30 form-control tokens by hand and
copied the body of form-control-frame() plus the :focus block, so a
change to the text-control frame never reached the OTP cell until
someone re-typed it. The cell now takes $form-control-tokens merged
with its own overrides (zero horizontal padding, cell width) and renders
through form-control-frame(); the 19 $form-otp-control-* variables that
only re-pointed a --cui-btn-input-* token are removed, leaving the width,
the horizontal padding and the size trios. Rendered values are
unchanged; the rule gains the eleven tokens the full map carries
(min-height, placeholder, disabled-*, action-*). Migration entry and the
OTP docs updated.
