refactor(forms)!: drop the v5 token names read as fallback hooks (#1085)

Three v5 custom-property names survived only as the first argument of a
var() nobody declared: --cui-input-placeholder-color inside the
--cui-control-placeholder-color default, --cui-input-focus-border-color
in the combobox focus rules and --cui-table-caption-color on the caption.
Each read now goes straight to the v6 source (--cui-fg-3,
--cui-btn-input-focus-border-color, $caption-color), the way every other
component already reads them. Same rendering; migration guide updated.
