docs(theme): document which slots pair with which (#1008)

Nine slots and nothing said which foreground belongs on which
background, so the combinations were left to guesswork — and reading a
foreground against a background it was not tuned for is where contrast
falls apart.

Adds the four pairings with a live example, and finishes the token
rename on this page: the slot column still listed text-emphasis and
border-subtle, which are fg-emphasis and border now, and the custom
colour example still used the old key. The base row names both emitted
spellings.
