fix(act): warn instead of error on self-healing failures (#1580)

Error-level logs are routinely wired to paging, and six sites reported
events that lose nothing and recover on their own — so healthy operation
paged someone.

Four are fire-and-forget cache writes, which are fire-and-forget precisely
because nothing depends on them: a miss costs a re-fold from the store. One
is a dropped metric sample. One is the dropped-ack report, whose own message
already says the work is redelivered.

Blocking, uncaught exceptions, the misdeclaration reports and the background
correlate failure stay at error: none of them resolve without a human.

Tests now assert the severity rather than leaving it incidental — the new
level fires and error does not, so this cannot drift back silently.
