fix: end a scan when the adapter becomes unauthorized or unavailable

A running scan only tore down on off/turningOff. Authorization can be revoked
from the settings app mid-scan and the adapter can disappear; in both cases the
native scan is dead while the stream stays open forever, waiting for
advertisements that can never arrive. unknown stays out — it is the state before
the adapter has reported itself, not one it falls back into.

scan_test covers all four states through the same case.
