Retry a chapter whose article fetch failed (#29)

A transient failure from the news article endpoint permanently dropped a
chapter. fetchInitialTopic returned the id even when FetchArticle failed,
so the first poll reporting that same id compared equal to activeID and
was skipped, and pollForChanges assigned activeID before fetching, so a
failed topic change was never revisited either.

fetchAndAddChapter now reports success and activeID advances only after a
chapter is appended, which leaves a failed id unaccepted and retried on
the next poll reporting it. The offset of the first observation is kept,
so a recovered chapter is placed where the topic actually started, and a
recovered initial topic still goes through the stale-topic check.
