* feat(config): resolve a project-level profile.md under the round's own

A round carrying no profile.md inherits ./.revmux/profile.md, and its bytes are
copied into the round as prompts/input-profile.md before anything reads them.
{{PROFILE}} names that copy, so a round records what calibrated it even after the
project file changes.

Resolution keys on projectDir rather than layers.project, which the
--config-dir ./.revmux collapse empties while the file stays on disk. Nothing is
written into input/: a snapshot there would be read as an explicit round-local
override on the next attempt. The TUI's profile tab reads the snapshot instead of
reconstructing the round-local path, which would have hidden it entirely.

* docs: make profile.md repo-scoped in fact rather than only in wording

references/task-dir.md called profile.md "reusable across the repo" and told the
compose step to copy it forward, while the design treated it as per-round context.
Both skill trees now say what actually happens, and the compose step no longer
generates one: it sees a diff and a file list, which cannot state a repo's
conventions, and a generated file wins over the project's.

Adds prompts/input-profile.md to all seven archive-tree diagrams and records in
CLAUDE.md that references/output.md in both trees is part of that set.

* fix: report a profile fallback that will not resolve, and stop on cancel

revmux config reported an unresolvable ./.revmux/profile.md as no fallback at
all, describing an invocation the review would refuse to start. It now carries
profile_fallback_error beside profile_fallback, as tasks_error and workdir_error
already do.

review checks ctx.Err() before materializing the snapshot: a signal arriving
between the claim and the write would otherwise burn a round nothing reviewed.
