fix(web): per-entry landing markers and compare-and-swap hardening (#2512)

Final hardening of the landing-marker protocol. Markers now carry
per-entry records ({id, text}) as the authoritative merge granularity:
concurrent tabs rewriting the marker UNION by entry identity (never
substring — a distinct occurrence can legitimately contain another
entry's phrase), appending only the entries whose ids they lack, with a
compare-and-revalidate write loop so two tabs reading the same marker
cannot silently drop each other's entries.

Generation counters carry a per-tab offset (hash of the tab id) on both
the counter and wall-clock paths, so two tabs picking the same successor
generation collide only on a 1-in-997 hash collision instead of letting
a refresh consume a newer landing as though it never arrived. A fresh
generation EPOCH clears the session's supersede record, which is keyed
by generation number and would otherwise suppress every backup of the
new epoch's lower-numbered landings. The flush announces a landing by
announced-id membership regardless of which tab's ack was the original,
so two tabs racing the same shared entry cannot re-announce an
already-merged transcript. Marker pruning is compare-and-delete: a fresh
marker another tab wrote between scan and removal is never deleted, and
this tab drops its own expired local landing state alongside its marker.

With this, the 12-PR stack reconstructs the fully-reviewed final state
of the voice-transcript outbox exactly.
