* feat(web): add inline rename action to the session info panel

* fix(web): settle in-flight rename on panel close, disable pencil while editing

Address codex P2 findings on #2489: closing the info panel mid-edit
unmounted the input before blur could commit, dropping the rename; and
re-clicking the pencil during an edit would blur-commit and reopen the
editor seeded from the stale title prop.

* fix(web): commit rename synchronously on close, optimistically patch active session view

Address codex round-2 P2s on #2489:
- settle an in-flight rename synchronously in the panel close paths
  (outside mousedown / Escape / trigger toggle) so a navigating click
  that unmounts the component cannot drop the edit;
- applyOptimisticSessionInfo patches sessionInfo in every live
  SessionStore where the session is active, so the chat header/info
  panel reflect a rename immediately instead of reverting until the
  daemon broadcast lands.

* fix(web): guard rename commit and active-view rollback against stale titles

Address codex round-3 P2s on #2489:
- the exposed commit() no-ops unless an edit is in flight, so panel-open
  paths cannot persist a draft seeded before an external title change
  (e.g. auto-title generation) as a user rename;
- the active-view rollback patches only stores whose title still equals
  the request's optimistic value, so a newer title that landed while the
  request was pending survives a late failure.
