* refactor(external-events): extract head-ref index maintainers into leaf [#941]

Module-decomposition increment 10: extract the per-cycle head-ref → PR-number
multimap index maintainers (add/remove; dedup + no-empty-key invariant) from
github-event-extension.ts into a new Pattern-B leaf github-pr-head-ref-index.ts.

Verbatim logic move; the legacy `headSha` misnomer is corrected to `headRef` on
extraction (the map was always keyed by the composite head-ref key, never a raw
SHA, so the rename changes no behavior). Adds 13 characterization tests covering
every branch — there was no prior direct unit coverage.

* docs(decomposition): record increment 10 PR number (#2474)
