fix(conventional-changelog,conventional-changelog-writer,conventional-changelog-conventionalcommits,template): use current host url paths (#1534)

GitLab removed the urls without the `-/` separator in 16.0, so every commit,
issue and comparison link generated for GitLab pointed at a legacy path.

Host configs now carry the comparison path as well, and both presets read the
issue path from the context instead of hardcoding it. Bitbucket issue links
follow along: its config still used the legacy singular `issue` path.

Fixes #986.
