feat(conventional-changelog-angular,conventional-changelog-conventionalcommits,template): group notes by their keyword (#1533)

Both presets rewrote the title of every note to `BREAKING CHANGES`, so a custom
parser `noteKeywords` entry — `SECURITY`, for example — ended up in the breaking
changes section and counted as a breaking change when recommending a version.

Notes are now grouped by their own keyword: breaking change spellings are merged
into a single group, any other keyword becomes a group of its own, and only
breaking changes affect the recommended bump. The conventionalcommits preset gets
`formatNoteTitle` and `formatNoteIcon` to rename groups and to put an icon in
front of them.

Fixes #815.
