feat(conventional-changelog-angular,conventional-changelog-conventionalcommits,template): format references in breaking change notes (#1529)

Issue references were formatted in commit subjects only, so `issuePrefixes`
and `formatIssueUrl` had no effect on breaking change notes.

Both presets now share `createReferencesFormatter` from the template package
and apply it to notes and subjects. It keeps already formatted Markdown as is:
links, bare urls, fenced code blocks and single-line code spans.

This changes existing output: notes with the default `#` prefix are linked now,
and references inside code spans, links and urls in subjects are no longer
replaced.

Fixes #631.
