feat(space): curated task milestone timeline (#2315)

* feat(space): curated task milestone timeline replacing raw actor-message log

Swap the task panel Timeline for a curated milestone feed. A new
taskMilestones.byTask LiveQuery unions content-rich rows from space_tasks
(creation + status transitions), sdk_messages (human instructions + agent
answers with real text, collapsed api_retry), workflow_run_artifacts
(PR/result/progress/review anchors), and space_github_events (CI/PR), dropping
handoff/thinking/hook/per-turn result noise. A new TaskMilestoneTimeline
component renders them tone-coded with real content, collapsing retry bursts,
deduping consecutive duplicates, and showing one timestamp per time bucket.

* fix(space): production-fidelity fixes for task milestone timeline (review)

Address PR #2315 review:
- P0: task:started referenced space_tasks.assigned_agent/agent_name, which do
  not exist in production (removed in migration 73) — broke the entire query.
  Source label is now NULL. Added a drift-guard test that prepares+runs the SQL
  against a fully-migrated DB so the harness can no longer mask column drift.
- P1: blocked tasks now render a warning "Blocked" milestone with block_reason
  (updateTask stamps completed_at on blocked, which previously rendered green).
- P1: GitHub milestones now source from the live space_external_events +
  deliveries tables (space_github_events is legacy with zero writes); check_run
  events render as danger since only failed conclusions are ingested.
- P1: failed human sends render as a distinct danger "Instruction failed to
  send" milestone instead of a normal instruction.
- P2: exact artifact-type matching (no %review%/%pr% substring collisions);
  $.review_url in the body chain; overwrite-style progress dated by updated_at;
  retry collapse bounded to a 5-min burst window; dedup is source-aware; the
  timeline re-renders every minute so relative timestamps refresh.
