docs: cover the v0.8.0 additions in README

The README missed everything from the roll-up/paint-on coalescing change: that
PR updated cue/doc.go and SPEC.md but not the README, so Coalesce,
CoalesceNone, TimedScreen.Mode, Decoder.Mode and -per-change were absent.

- Decoding: Mode(), plus the incremental-Feed guarantee -- successive Feed calls
  form one continuous stream, which doubled control codes and extended
  characters both depend on.
- Timed-text cues: why the direct-write modes need a coalescing rule at all,
  the two Coalesce values, why a period is stamped at its first change rather
  than at completion, and why the rule is gated on the caption mode.
- Command-line tools: a table for -no-preroll and -per-change, and the SCC vs
  WebVTT timing-convention note that explains why SCC -> WebVTT -> SCC now
  returns the original timecodes.
