docs: add the comment brevity and review severity rules

Comments and docs are a liability, not a deliverable. A comment earns its
place only by stating what the code cannot, and never by narrating a
change's own history. Comments in tests are rarer still: the test name
states the behavior and the body shows the setup, so almost every test
comment is a restatement.

Review severity tracks user-visible consequence, and a comment or
documentation inaccuracy is never critical or major - it cannot break
anything at runtime. A review round that comes back mostly documentation
findings means the code is done and the prose is overgrown.
