feat: add SSID scan dropdown, hostname field, and refresh to the default provisioning form (B1-809) (#1117)

- hostname input (optional, maxlength=32) with an mDNS-name hint.
- SSID <select> populated from POST /api/wifi/scan ({"aps","state"}
  shape shipped in #1114); picking an option fills the always-present
  manual SSID text input, which is the sole "ssid" field POSTed --
  hidden networks and scan failures still work via manual entry.
- Refresh button re-triggers POST /api/wifi/scan?refresh=1, labelled to
  warn that refreshing briefly disconnects the client (single-radio
  channel hop, unavoidable per bb_wifi_prov's own route doc).
- uses the "state" field: "pending" shows a scanning affordance,
  "ready" with an empty list shows "no networks found", distinct from
  "not_started" showing nothing yet.
- vanilla JS only, no external resources; the manual SSID/password/
  hostname inputs and the form's plain POST still work if JS never
  runs or the scan fetch fails.

Gzipped asset: 405 -> 1298 bytes.
