Recognize month and datetime-local widget subclasses as form-control widgets (#849)

Django ships no built-in widgets for these HTML5 input types, but the
standard recipe (subclassing DateInput/DateTimeInput and overriding
input_type) produces widgets is_form_control_widget didn't recognize,
so they silently missed out on addons and floating-label support.

Fixes #309, #678. #740 ("date-local") isn't a real HTML5 input type,
so not adding it here.
