Add layout setting for default form/field layout (#840)

* Add default_layout setting

* Add tests

* Rename default_layout setting to layout for naming consistency

Every other BOOTSTRAP5 setting shares its name with the kwarg it
defaults (wrapper_class, field_class, checkbox_layout, ...); update
CHANGELOG and cherry-pick attribution for #531.
