docs(generate): note that discontinuity recovery is a receiver concern

The stale-caption case after a seek is resolved by the player resetting its 608 state at a discontinuity, as it would any other decoder. Say so in the WithFlipAtCueStart docs and the README, where the wording so far stopped at what the sender cannot do.
